Personal Healthcare Providers is a concierge practice.  As such, there is an annual administrative fee charged to all patients.  This annual fee allows our providers to have a small number of patients and increased availability.  It also allows for a dedicated staff to help patients with administrative and care coordination services.  In addition to the annual fee, our providers charge separately for “covered services”.  Covered services are direct medical services that are traditionally reimbursed by third party payors and designated by a CPT code.  While we do not participate with private insurance companies, we will submit claims on your behalf and advocate for reimbursement of these covered services.  If you are a Medicare patient, please note we are “opted-out” of Medicare and neither our practice nor you as a patient can submit any claim for reimbursement of covered services to Medicare.
